Virginia Department of Motor Vehicles (DMV) Forms

A visit to the Virginia DMV can be significantly easier if you take advantage of the Virginia DMV’s online document library, where all the necessary forms for their services are available for download. While each of these forms can also be picked up in person at any Virginia DMV branch, having the ability to print them ahead of time can save time and stress from a DMV visit. By knowing which forms are required for your needs at the DMV, you can expedite your time by filling them out in advance, making things a little easier for everyone involved.

The Virginia Drivers Manual

The Virginia Drivers Manual is your complete guide to all things related to driving, the DMV, and traffic laws. It is available in both printed and electronic form, and makes for the single easiest way to learn about the policies of the DMV, the traffic laws in Virginia, and best practices for responsible driving. Out-of-state drivers, new drivers, holders of suspended licenses, and anyone else who needs to familiarize themselves with driving in Virginia should regularly consult the driver’s handbook to be fully prepared on the road.

Driver License and Identification Card Application (Form DL-1P)

The first step to becoming a legal driver in Virginia is getting a drivers license. If you’re looking to get a drivers license or an identification card, you need to complete a Driver License and Identification Card Application (Form DL-1P). On this form, you’ll select the type of license you are applying for and provide your personal information. You can also use this form to simultaneously register as an organ donor. Once you’ve passed all the necessary examinations and road tests, you’ll be on your way to driving legally.

Address Change Request (Form ISD-01)

Whenever an important piece of information about you changes, such as your name or address, it is vital that you inform the DMV as soon as possible. The Address Change Request Form (Form ISD-01) is the official way to let the DMV know that you are now living in a new location. After supplying the DMV with your new mailing address, you will be provided with an official address card to accompany your driver’s license. The next time you renew your license, your new card will bear your current address.

Disabled Parking Placard or License Plates Application (Form Med-10)

The Virginia DMV assigns handicap parking placards and plates based upon criteria of mobility impairment, as determined by a medical professional. If you and your doctor complete a Disabled Parking Placard Application (Form Med-10), the DMV will mail you a placard which can be hung from your rear-view window and qualify you to park in specially-designated spots around the state and country. Permanent handicap license plates can also be installed in cases of permanent or long-term impairment, again with the discretion of a license medical professional.

Application for Certificate of Title and Registration (Form VSA-17A)

Titling and registering a car after you purchase it is vital to establishing proof of legal ownership of the vehicle. Fortunately, you can accomplish both of these using the same form, the Application for Certificate of Title and Registration (Form VSA-17A). You will need to keep your car registered annually (which can be done online), but the initial registration must be performed in person at a DMV office branch. Your certificate of title serves as proof of legal ownership of the vehicle, so keep it safe. Driving without a current registration can lead to fines and loss of your vehicle, so be extra sure to keep it up to date as well.
