Arizona Department of Motor Vehicles (DMV) Forms

The Arizona MVD carries an extensive library of official forms needed for all its services. The convenience of being able to access them online means that you can research exactly what information is required for each of them, complete them before going to the local branch, and save yourself a lot of time and stress that could otherwise arise. Each form requires very specific information to be processed, so take a little time to check out the most common ones you will encounter. You’ll be able to handle all your driving needs and services much more efficiently when you are familiar with the various paperwork involved.

The Arizona Driver License Manual

If you’ve ever wanted one central document on all things related to driving in Arizona, the driver license manual was made with exactly that goal in mind. It explains in a concise manner the most relevant information about how the MVD operates, what you have to do to start driving, and how you will be expected to behave on the road. You’ll also glean a lot of information about the processes of license renewal, vehicle registration, and the many other administrative tasks that the MVD handles. Download a copy in PDF form, or pick one up at any MVD branch location.

Application for Title and Registration (Form 96-2036)

The first step to owning a car after completing the transaction with the seller is to title and register it with the MVD using an Application for Title and Registration (Form 96-2036). In fact, it’s so important that neglecting to do it soon after the purchase date can cause significant late fees. Acquiring a title in your name indicates legal proof of ownership, and is very handy in the event you should wish to transfer ownership of the vehicle in the future. Additionally, your registration must be renewed every year to be valid, though subsequent annual registrations can be done very easily online without having to take a trip to an actual MVD branch.

Bill of Sale (Form 48-2004)

A proper Bill of Sale (Form 48-2004) may seem unnecessary when purchasing a vehicle, but it’s actually a very important form of protection for both parties involved. It serves as a valid record of the transaction between the parties involved, recording information about the status of the car and the date of the transfer of its ownership. Sellers often use it to protect themselves against any illicit conduct a new buyer may perform with the car shortly after purchase, and buyers can use it to protect against fraudulent claims about the state of the car at the time of purchase. It’s definitely worth your time to print out the bill of sale form and have both people complete and sign it anytime a vehicle sale occurs.

Application for a Handicap Parking Placard or Plate (Form 96-0104)

If you suffer from a legitimate mobility impairment problem, you may qualify to park in specially-designated handicap parking spots which are conveniently located close to the entrances of many buildings. To acquire a special plate or placard which enables you to use this feature though, you and your doctor need to complete the proper application and turn it in to the MVD. Your doctor will need to state the nature of your condition and whether it is permanent or temporary. Upon approval, the MVD will grant you your placard or plate for the length of time deemed worthwhile by your doctor’s endorsement.
